The hospice team at CHI Health at Home is comprised of a professional interdisciplinary team that is focused on ensuring each client is provided the individualized care that he/she deserves. These professionals are trained in various disciplines and collaborate to ensure the delivery of the best hospice care possible. This team meets regularly to coordinate the care needs of the whole person; to provide physical, spiritual, and emotional comfort. This interdisciplinary team consists of the following:
- Medical Directors: work with your personal physician to assure that the physical aspects of your care are closely monitored and attended so that the best possible comfort care can be achieved.
- Registered Nurses and/or Licensed Practical Nurses: visit routinely to assess and monitor your physical condition. Nursing services will include medication management, education and support to the client and caregivers. The Hospice nurse is on-call 24 hours a day, seven days a week.
- Hospice Aides: assist with personal care, light housekeeping and companionship during intermittent visits throughout the week.
- Social Workers: offer emotional/anticipatory grief support, counseling, coordination of community resources and support services, to include but not limited to volunteer and spiritual coordination, as well as helping facilitate for final arrangements.
- Chaplains: will provide spiritual care directly, or coordinate visits with other clergy according to your wishes and assist with end of life arrangements.
- Volunteers: are men and women of all ages with varying skills and talents. Volunteers are specially trained to provide companionship, assistance and additional support to the client and caregiver. Examples may include card playing, writing letters, light housekeeping or any additional identified needs. Volunteers will visit upon request, based on availability.
- Bereavement Coordinators: provide ongoing grief support to caregivers/family members for 13 months. Home visits, phone calls, grief support groups, and mailings are among the many services offered. Hospice staff is available to talk with clients, family members and facility staff about the grief and loss you might be experiencing. Books and other reading materials are available. Hospice has specially designed bereavement programs for people of all ages.
