In order to qualify for home health services with CHI Health at Home, there are some basic qualifications to consider:
- Individuals that have difficulty leaving their home unassisted or without an assistive device
- Individuals in need of continued skilled care and/or therapy to reach goals
- Individuals with the potential to improve function or achieve heightened independence
- Individuals with:
- new or complex diagnosis
- new or frequent medication changes
- frequent falls or safety concerns
- recent hospitalization
- need for IV therapy or infusions
