The Home Health Team

Your home health care team will be comprised of highly trained medical professionals, dedicated to getting you well. We will come to your home per your doctor’s orders, listen to you, assess your unique needs and discuss the best plan of care with your physician to restore your quality of life. Our team meets regularly to coordinate your individual needs. This team consists of the following disciplines:

  1. Skilled Nursing – RN/LPN
    Our nurses collaborate with each patient’s physician to provide one-on-one care and education to help patients improve their health faster and recover in the comfort of their own home. We provide care based on our patient’s individual treatment plan through the highest levels of quality, compassion and respect.
  2. Physical Therapists (PT/PTA)
    Physical Therapists are specially trained in maximizing a client’s ability to be safe at home. They will assess your strength, range of motion, balance, endurance and functional movements such as ambulation and transfers within the home. Assessments will include a review of your living quarters to minimize safety hazards and reduce the risk of falls.
  3. Occupational Therapists (OT/COTA)
    Occupational Therapists are specially trained in maximizing a client’s ability to perform the activities of daily living such as bathing, grooming and dressing. Assessments will include your need for adaptive and safety equipment throughout your home.
  4. Speech-Language Pathologists (SLP)
    Speech-Language Pathologists are specially trained in evaluating and treating swallowing, communication disorders, and cognitive impairments.
  5. Medical Social Worker
    Medical Social Workers provide short-term counseling services, coordination with community resources, long-range planning, healthcare directives and assistance with finances or alternative living arrangements if necessary.
  6. Home Health Aides
    Home Health Aides have experience and training in providing care in the home. An aide is assigned when there is a specific need for personal care on a part-time basis at home.
